I have an older MacBook and I suddenly had this problem .I tried all the solutions  on your list but nothing work . I had always suspected it was a bug in a MacOS  upgrade because it completely disable the bluetooth and the OS level.   I called Apple and they said the reason was my bluetooth hardware had failed.    Last week I installed Parallels and Windows on my MacBook and suddenly my bluetooth came back to life. As I had suspected something in MacOS corrupted the bluetooth at the OS driver level but installing Windows reinstalled a different bluetooth driver . Now it is 100% working again after not working for months and months .  I wish someone else would try this solution and verify that it works]]></description>
